چکیده

The intersection of deteriorating patients, Early Warning Scores, Rapid Response Teams and new monitoring technology These days, hospitals are treating increasingly complex patients with multiple co-morbidities. At any given time some of these patients may be rapidly deteriorating, for a variety of reasons. Every hospital must have a strategy to identify such patients, and be capable of providing the appropriate level of care at the right time. Early intervention on a patient who is deteriorating is likely to improve that patient’s outcome.1,2

Characteristics and outcomes of pediatric rapid response teams before and after mandatory triggering by an elevated Pediatric Early Warning System (PEWS) score.

BACKGROUND The Pediatric Early Warning System (PEWS) was created to identify unstable patients before their deterioration. Rapid response teams (RRTs) were developed to assist with management of such patients. In 2009, our institution mandated the activation of RRTs if a PEWS score was elevated (ie, ≥5). OBJECTIVES The goal of this study was to examine changes in characteristics of RRT calls ...

Improving Patient Safety through the Use of Nursing Surveillance.

Surveillance and monitoring each represent a distinct process in patient care. Monitoring involves observation, measurement, and recording of physiological parameters, while surveillance is a systematic, goal-directed process based on early detection of signs of change, interpretation of the clinical implications of such changes, and initiation of rapid, appropriate interventions. Through use o...

Risk stratification of hospitalized patients on the wards.

Patients who suffer adverse events on the wards, such as cardiac arrest and death, often have vital sign abnormalities hours before the event. Early warning scores have been developed with the aim of identifying clinical deterioration early and have been recommended by the National Institute for Health and Clinical Excellence. In this review, we discuss recently developed and validated risk sco...

Can binary early warning scores perform as well as standard early warning scores for discriminating a patient's risk of cardiac arrest, death or unanticipated intensive care unit admission?

INTRODUCTION Although the weightings to be summed in an early warning score (EWS) calculation are small, calculation and other errors occur frequently, potentially impacting on hospital efficiency and patient care. Use of a simpler EWS has the potential to reduce errors. METHODS We truncated 36 published 'standard' EWSs so that, for each component, only two scores were possible: 0 when the st...

Early Warning Scoring System for Early Recognition of and Timely Intervention in Deteriorating Patients in the Hospital.

Delay in recognizing clinical deterioration in acutely unwell patients often leads to cardiac arrests and unexpected admissions to the Intensive Care Unit. Early Warning Scoring Systems based on vital physiological parameters help in early detection of clinical deterioration. When combined with a system of prompt and appropriate clinical response, they have been shown to improve outcome. A stan...
